Training health care professionals is only the first step towards an effective and sustainable self-management culture. Trained individuals require continuous contact to nurture, support and maintain the knowledge gained during training. Along with sustainable programming, we also strive to achieve sustained expertise to ‘implement self-management supports. One on one Behaviour Change training and support is one to way to achieve this expertise in the Champlain LHIN.

A team of Trained Health Professionals will be able to provide on-site training for your organization.
The C&C workshop is accredited by the CFPC for 1 Mainpro-M1 credit for every hour of CME, up to 6 credits in total.
There are a number of different agendas to choose for the C&C workshop- ½ day workshop is 4.5 hours, the full day is 6.5 (with lunch in the middle); there are also 4 1-hr modules and 2 2-hour modules
