Peer Leader Training Opportunity!

The “Living a Healthy Life with Chronic Conditions” workshop helps people with chronic health conditions to better manage their symptoms and their daily life. It is FREE to participants, and participants receive the Living a Healthy Life with Chronic Conditions reference book for free. All participants must pre-register for a workshop. We suggest participants attend all six sessions if possible. The more sessions attended, the more benefit!

The workshop is led by two leaders who have taken special training. The leaders are volunteers or health care professionals, many of whom have chronic conditions themselves
 
Peer Leader Training is four 8-hour days.  This training will consist of two days the first week and two the following week. Training will be held in English.
 
Leaders are individuals interested in teaching and empowering others to better manage their chronic conditions and to live a healthy life. As a leader, you will have the satisfying task of integrating the contents of the program and learning group facilitation skills that can be transferred to other areas of your life. Most importantly, you will watch people affected by chronic conditions successfully manage their health conditions by using the skills and tools that you have shared with them.
Leaders may be lay people or health care professionals. Regardless, they act as peers when leading the workshop, not as health experts. Two Peer Leaders always work together to deliver the workshops.

The Training session will be held in English, however we can provide French Peer Leader manuals to those who wish to facilitate the workshops in either language.

A team of Trained Health Professionals will be able to provide on-site training for your organization.
 
The C&C workshop is accredited by the CFPC for 1 Mainpro-M1 credit for every hour of CME, up to 6 credits in total. 

There are a number of different agendas to choose for the C&C workshop- ½ day workshop is 4.5 hours, the full day is 6.5 (with lunch in the middle); there are also 4 1-hr modules and 2 2-hour modules, and a 1 hour Lunch and Learn Session

Lunch and Learns

If your organization is interested in implementing self-management into practice a lunch and learn can be very beneficial to ensure a consistent and coherent understanding and approach to self-management.

These sessions can also be highly beneficial for those organizations that may have already received some training in self-management, and/or are currently running the Living a Healthy Life with Chronic Conditions workshop.

A needs assessment will allow Living Healthy Champlain to identify your organization’s current approach to implementing self-management and support overcoming some barriers that could prevent  you from  utilizing the skills you have learned.
